3M Lava vs. Ivoclar E-Max: Which is the Strongest?

Have you given much thought to the question of which is stronger, 3M Lava or E-Max? It may not be on the top of your list of life's questions to ponder, but given that your patients want a product that will stand up to the rigors of their oral environment, it is worth taking a few minutes to consider the pros and cons of the metal-free products known as Zirconium (3M Lava) and Dicillicates (Ivoclar's E-Max). At our lab, we've put them both to the test and have some valuable insight based on our considerable experience of working with both products.

Any new product should be evaluated based on the factors of fit, strength, esthetics and general performance in an oral environment over time. 3M Lava has been available in the US for six years and even longer internationally. Ivoclar's E-Max has been available for three years. Of course, what will determine the long-term viability of these two products is their ultimate performance in the mouth.

Referred patients usually have fewer objections and are also more likely to be long-term, loyal patients. While most dental practice owners won’t dispute these facts, they will dispute what the primary factor is in motivating a patient to refer a friend to the practice. Most clinicians would like to believe that it is their exceptional clinical skills. And yet, both research and experience indicate that with most patients, this is not the case. Dr. Aldo Leopardi, Founder of the Denver Implant Study Club, Life Adventurer and Rock Star!

When Dr. Aldo Leopardi speaks, dentists' listen. A much sought-after national speaker, Dr. Leopardi is very knowledgeable on all aspects of implant therapy. His Greenwood Village-based practice offers comprehensive prosthodontic services, including fixed crown and bridge, removable prosthodontics, implant therapy, aesthetic services and temporomandibular joint therapy. His boundless energy and full-throtle, adrenaline-fueled practice has placed him at the top of the prosthodontic field. Plus, he's a rock star, but more about that later.

Thankfully, Dr. Leopardi is happy to share what he knows with his constituents, and to that end, he founded the Denver Implant Study Club (DISC) in 2003. This study club is unlike any other. It consists of a series of lectures with real-world scenarios that are applicable across a broad range of dental implant systems.

In 1996, he joined the faculty at the University of Detroit Mercy Michigan, where he also earned his American DDS degree. After 15 years of combined academic and dental practice, Dr. Leopardi resumed private practice limited to implant, fixed and removable dentistry in Denver.

  Perry & Young Case Study

This patient went to her dentist requesting implants. Her case required cement on crowns and because of our extensive experience in crowns, her doctor contacted us. After following protocol and the succesful completion of the procedure, the patient appeared happy with the results. But she received some negative feedback from her husband, which sent her back to her doctor to see if there were other options that would improve the asethetic outcome. We went back to the drawing board and set out to provide her with an aesthetic solution without compromising the secure adhesion of cement. We determined that lingual set screws were the best choice for this particular case.

Many of our dentist partners call Perry & Young about large implant cases to ask how to design them so they are both retrievable and aesthetic. We have had incredible results implementing lingual set screws for our large cases. The lingual set screw is designed to engage a dimple on the lingual of the abutment and secures the unit to the implant abutment. Our technicians take great care to plan and to carefully tap and thread the copings (as shown in the lab photos).

This technique gives the clinician the opportunity to adjust or repair the bridges, out of the mouth, by simply turning the set screws and removing the units needing to be worked on. Typically, the clinician can create a hermetic seal using a small amount of light body impression material and placing the bridge onto the abutments and tightening the set screws. There is an additional cost to doing this, but our patients are happy to have no dark spots in the occlusals of their implant-supported restorations, making this an aesthetically pleasing option for implants.

The referring doctor and the patient were pleased with the final results and the patient feels great about her smile. Success!
